SERVICES WE OFFER:
Case Management; is an activity under which responsibility for locating, coordinating
and monitoring a group of necessary services for a participant rests with
a designated person or organization in order to promote the effective and efficient
access to necessary comprehensive health services. Case management seeks to
promote the good health of participants and includes referral to various agencies for
other needed services.

Healthy Children and Youth (HCY) Program; in Missouri is a comprehensive,
primary and preventive health care program for MO HealthNet eligible children and
youth under the age of 21 years in covered eligibility groups.

Personal Care Services ; offers medically related services designed to meet the maintenance needs of our clients with chronic, stable condition. This service provides two(2) types of services.
Personal Care task include but not limited to; Planning, preparation , & clean-up of meals; Making beds and changing sheets; Brushing, combing, shampooing hair; Giving bed baths and assisting with other baths; Brushing teeth, cleaning dentures; Assisting to and from bed, wheelchair, walker, or chair; Assisting with medications; Shopping for groceries or household items.
Advanced Personal Care task include but not limited to; Removing external catheters, inspect skin and reapplication; Using a lift; Manually assist with oral medications which are set up by a RN or LPN; Provide Passive range of motion; Applying non-sterile dressings to skin breaks.
Homemaker/Chore Services include but not limited to; Assisting with Light housekeeping; Cleaning bathroom; Cleaning kitchen; Assisting with Laundry; Washing Dishes; Cleaning medical equipment; Cleaning floors.
